Welcome to Brian Thomasen NP in Psychiatry. We are committed to providing high-quality outpatient psychiatric care to children, adolescents, young adults, and their parents. Please carefully review the following practice policies, as they outline important aspects of our services.

Consent to Treatment:

  • Participation in psychiatric evaluation and treatment at Brian Thomasen NP in Psychiatry requires the voluntary consent of the patient or, if the patient is a minor or legally unable to provide consent, their legal guardian.

  • By engaging in our services, patients (or their legal guardians) acknowledge that they understand the nature and purpose of the psychiatric evaluation and the subsequent treatment recommendations, which may include medication management and referrals. This consent implies an understanding that the goal of treatment is to address mental health concerns and promote well-being.

  • Patients have the right to ask questions about the evaluation process, proposed treatment plans, and any alternative options. Active participation in decision-making regarding their care is encouraged.

  • Please note that the initiation of any medication will involve a more detailed and specific informed consent process, as outlined in the "Medication Management" section of these policies, ensuring a thorough understanding of the medication's risks, benefits, and alternatives.

  • Your engagement in our services signifies your consent to receive psychiatric evaluation and treatment in accordance with these policies and the professional judgment of Brian Thomasen NP.

  • Patients (or their legal guardians) have the right to withdraw consent for treatment at any time. To ensure continuity of care, we encourage discussing this decision with Brian Thomasen NP to explore alternative options or facilitate referrals as needed.

Scope of Practice:

  • Brian Thomasen NP in Psychiatry specializes in medication management and psychiatric evaluations.

  • Our practice serves individuals between the ages of three (3) and twenty-eight (28) years old.

  • In certain circumstances, we may provide medication management to parents of patients who are also receiving treatment with our practice.

  • For patients approaching or exceeding the age of 28, Brian Thomasen NP will collaborate with the patient and/or their legal guardian to develop a transition plan which may include referrals to appropriate providers. Continued care with our practice may be considered for established patients on a case-by-case basis, subject to clinical appropriateness.

Requirement for Concurrent Therapy:

  • A primary condition for receiving treatment at Brian Thomasen NP in Psychiatry is that all patients must be actively engaged in ongoing therapy with a licensed mental health therapist.

  • You will be required to provide a letter from your therapist confirming your current participation in therapy prior to your initial appointment.

  • This therapist confirmation letter must be resubmitted every three (3) months to ensure continuity of care. For patients demonstrating consistent stability, as determined by Brian Thomasen NP, this requirement may be adjusted to every six (6) months at the provider’s discretion. Treatment may be paused or discontinued if this documentation is not provided. It can be delivered by email from the therapist or uploaded through the Simple Practice Client Portal.

Payment Policy:

  • Brian Thomasen NP in Psychiatry is a self-pay practice and does not accept insurance.

  • Payment is due prior to the start of each session.

  • We accept cash, credit card, debit card, and HSA/FSA. Checks should be made payable to "Brian Thomasen NP in Psychiatry PLLC".

  • Cash Payment Discount: Patients paying in cash will receive a 15% discount on all service fees.

  • Our current fees are as follows (prices are subject to change with advance notice):

    • Medication Management (30 minutes): $299 (Cash Price: $254.15)

    • Psychiatric Evaluation: $599 (Cash Price: $509.15)

  • In cases of financial hardship, patients or their legal guardians are encouraged to discuss payment concerns with Brian Thomasen NP in advance of their appointment. Limited payment plan options may be considered on a case-by-case basis, subject to approval.

Required Documentation:

  • Therapist Confirmation Letter: As outlined above, a letter from your current therapist confirming ongoing treatment is required at the start of treatment and every three (3) months thereafter.

  • Physical Examination Record: Patients must provide a copy of their most recent physical examination record from their primary care provider. This documentation helps ensure comprehensive and safe treatment.

  • State Identification: Patients or their adult caregivers must provide a copy of their valid State-issued Identification Card.

  • HIPAA Release: Patients or their adult caregivers will be required to sign a HIPAA Release form allowing Brian Thomasen NP in Psychiatry to collaborate with the patient's primary care provider, specialist providers, and therapist to ensure coordinated care.

  • All required documentation (therapist confirmation letter, physical examination record, state identification, and HIPAA release) should be submitted electronically via the Simple Practice portal or by email to Brian@bebetterbrian.com. If documentation is not received by the specified deadlines, appointments may be rescheduled until requirements are met, to ensure safe and coordinated care.

Appointment Format:

  • Psychiatric Evaluations: Initial psychiatric evaluations will be conducted in the patient's home or at another designated location agreed upon with the provider.

  • Follow-Up Medication Management Appointments: Routine follow-up appointments will be conducted virtually using the Simple Practice telehealth portal via the internet, unless an alternative arrangement is made and agreed upon in advance.

  • While routine follow-up medication management appointments are typically conducted virtually, in-person follow-up appointments may be arranged on a case-by-case basis if clinically necessary or preferred by the patient, subject to mutual agreement with Brian Thomasen NP.

Communication:

  • All patients and families should call the practice phone number 718-218-4134 for routine inquires during regular business hours. When leaving a voicemail message, include the patient’s name, date of birth, a phone number, and a brief description of what is being requested.

  • Patients may choose a preferred method of communication (email, phone, or secure texting via a HIPAA-compliant platform, if available). Texting is limited to non-clinical inquiries (e.g., appointment scheduling) and will be responded to within 1–2 business days, consistent with email.

  • Patients and their adult caregivers may email the practice at Brian@bebetterbrian.com and should allow for one to two (1-2) business days for a response. Routine email inquiries will be addressed during regular business hours.

  • For psychiatric or medical emergencies, please visit the nearest emergency room or call 911 immediately.

Prescription Refill Policy:

  • Medication refills will be provided once the patient has demonstrated stability on their current medication regimen, as determined by Brian Thomasen NP.

  • Refill requests must be submitted at least five (5) business days before the patient is expected to run out of medication to allow sufficient time for processing and transmission of the prescription.

  • Refill requests should be submitted by calling 718-218-4134 or emailing Brian@Bebetterbrian.com. Requests should include the patient’s name, medication name, dosage, and pharmacy details.

Cancellation and "Did Not Show" (DNS) Policy:

  • Patients and their families are required to call 718-218-4134 to cancel or reschedule appointments at least 24 hours prior to the scheduled appointment time.

  • Failure to provide at least 24 hours' notice will result in the appointment being recorded as a "Did Not Show" (DNS).

  • Patients who present for their scheduled session more than 15 minutes late will also be considered a DNS.

  • Patients will be billed the full session fee for all DNS appointments.

  • Exceptions to the DNS fee may be considered for documented emergencies (e.g., sudden illness or family crisis) at the discretion of Brian Thomasen NP. Patients are encouraged to notify the practice as soon as possible in such cases.

Termination of Treatment Policy:

  • Treatment at Brian Thomasen NP in Psychiatry may be terminated if a patient misses three (3) scheduled appointments within a 90-day period or for other reasons, such as non-compliance with treatment requirements or safety concerns, determined on a case-by-case basis.

  • In the event of termination, patients will receive written notice via email or mail, including the reason for termination and a 30-day period (where clinically appropriate) to continue medication management for transition purposes. Brian Thomasen NP will provide referrals to other providers to support continuity of care, upon request.

Inclement Weather Policy:

  • In the event of inclement weather, scheduled in-person psychiatric evaluations will be rescheduled for the next available appointment, and patients will be notified via phone (718-218-4134) or email (Brian@bebetterbrian.com) at least 24 hours in advance, when possible. If inclement weather causes power or internet service interruptions affecting telehealth appointments, the appointment will be rescheduled, or, if feasible, conducted via an alternative secure platform (e.g., phone). Patients will be informed of the new appointment details promptly.

Professional Boundaries:

  • Your well-being is our priority, and we maintain professional boundaries to ensure objective, effective care. Our interactions will remain within the therapeutic context, focusing on your mental health needs in a safe and ethical manner. We appreciate your cooperation in upholding these guidelines.

Grievance Policy:

  • At Brian Thomasen NP in Psychiatry, we are committed to providing the highest quality of care. We understand that there may be times when patients or their families have concerns or complaints about our services or policies. We encourage you to share these with us so that we can address them and improve our practice.

  • Process for Filing a Grievance:

    1. Verbal Communication: We encourage you to first attempt to resolve any concerns directly by speaking with Brian Thomasen NP. Often, misunderstandings can be resolved quickly through open communication.

    2. Written Grievance: If the issue is not resolved through verbal communication, or if you prefer to submit your concerns in writing, please do so. Your written grievance should include the following information:

      • Your name (or the patient's name, if you are submitting on their behalf)

      • Date of the incident or issue

      • A clear and concise description of the concern or complaint

      • The names of any individuals involved

      • Your preferred contact information (phone number and/or email address)

      • Any desired resolution

Written grievances can be submitted via email to Brian@bebetterbrian.com.

Acknowledgement of Grievance: Upon receipt of your written grievance, we will acknowledge it within 3-5 business days.

    1. Review and Investigation: Brian Thomasen NP will review and investigate the grievance thoroughly and impartially. This may involve gathering information from relevant parties and reviewing any pertinent documentation.

    2. Response and Resolution: We will make every effort to address your concerns fairly and in a timely manner. You will receive a written response outlining the findings of our review and any steps taken or proposed to resolve the issue within 7–10 business days for minor concerns or 10–15 business days for complex issues, following the acknowledgement of your grievance.

    3. Further Action (Optional): If you are not satisfied with the resolution provided by Brian Thomasen NP in Psychiatry, you may have the option to pursue further action through external regulatory bodies or professional licensing boards. Information about these resources can be provided upon request.

  • Zero Tolerance for Aggression: Verbal or physical aggression directed towards Brian Thomasen NP will not be tolerated. Any instance of assault or damage to property will be reported to the appropriate authorities, and legal prosecution will be pursued to the fullest extent of the law.

  • Confidentiality: All grievances will be handled with sensitivity and respect for your privacy. Information shared during the grievance process will be kept confidential to the extent possible, consistent with a thorough investigation and resolution.

  • Non-Retaliation: Brian Thomasen NP in Psychiatry assures that there will be no retaliation against any patient or family member for filing a grievance in good faith.

Record Keeping:

  • Brian Thomasen NP in Psychiatry maintains clinical records for each patient, including psychiatric evaluations, progress notes, and other relevant information related to your treatment. These records are kept securely using the Simple Practice electronic health record (EHR) system, which is compliant with HIPAA regulations to protect the privacy and security of your Protected Health Information (PHI).

  • Patients have the right to request access to their records in accordance with HIPAA guidelines.

  • Requests should be submitted in writing via email to Brian@bebetterbrian.com, and records will be provided within 30 days, with potential fees for copying or processing as permitted by law. Additional information about this process can be provided upon request.

Insurance and Billing:

  • Brian Thomasen NP in Psychiatry is a self-pay practice and does not accept insurance directly.

  • For the purpose of completing prior authorizations for prescribed medications, patients will be expected to provide their insurance card information as needed.

  • Patients will be provided with an invoice for services rendered, which they may choose to submit to their insurance company for potential out-of-network reimbursement. Please note that reimbursement is not guaranteed and is dependent on your individual insurance plan.

  • Brian Thomasen NP in Psychiatry does not handle the process of submitting claims to insurance companies or obtaining prior authorizations (other than the administrative completion of necessary forms with patient-provided insurance information).

  • Brian Thomasen NP in Psychiatry will provide a superbill for services rendered and monthly financial statement via e-mail, which includes detailed billing codes and information necessary for patients to submit to their insurance company for potential out-of-network reimbursement.

Approach to Care:

  • At Brian Thomasen NP in Psychiatry, our approach to care is highly individualized and collaborative. We believe that mental health goals and needs can evolve, and therefore, treatment is tailored to the unique circumstances and immediate concerns of each patient and their family.

  • While formal, long-term treatment plans may not be the primary focus, we engage in ongoing discussions with patients and their families to understand their current challenges, desired outcomes, and preferences for medication management. Progress is assessed through regular symptom tracking, patient and family feedback, and clinical evaluations during appointments.

  • This continuous dialogue informs our decisions and ensures that care is responsive and relevant to the patient's needs in the moment. We value the active participation of patients and their families in shaping their treatment journey.

Medication Management:

  • Prior to initiating any medication, Brian Thomasen NP will provide comprehensive informed consent in session. This will include a discussion of the medication's risks, benefits, alternative treatment options, and potential side effects. Patients and/or their adult guardians will be provided with medication information from MedlinePlus (https://medlineplus.gov/), a reliable resource from the United States Government available in both English and Spanish. A signed informed consent document will be obtained, certifying understanding of this information.

  • Patients and their families are encouraged to ask any questions they may have about their medications during sessions and can also call the provider with questions on demand.

  • Reporting Side Effects: Any side effects experienced while taking medication should be reported to Brian Thomasen NP during scheduled sessions. For more serious side effects or adverse reactions, patients should call the provider immediately.

  • Urgent Medical Attention: Certain serious side effects, such as those associated with Neuroleptic Malignant Syndrome or Tardive Dyskinesia, require immediate medical attention. In the event of such symptoms, patients should proceed to the nearest emergency room.

  • Discontinuation of Medication Due to Side Effects: Patients have the right to discontinue any medication at any time if they experience side effects that are intolerable. However, it is strongly recommended that they discuss this decision with Brian Thomasen NP as soon as possible to ensure safe management and explore alternative options if needed.

  • Changes in Medication: Medication adjustments are an integral part of the treatment process and are made based on a careful evaluation of the patient's response, ongoing symptoms, and any emerging side effects. Decisions regarding changes to the medication regimen (including dosage adjustments, switching medications, or discontinuing a medication) will be made collaboratively with the patient and, when appropriate, their adult caregivers.

    • Communication is Key: Any proposed changes in medication will be discussed thoroughly in session, including the rationale for the change, potential benefits, and possible risks or side effects. Patients and their families are encouraged to ask questions and share their perspectives and concerns.

    • Factors Influencing Change: Medication changes may be considered due to:

      • Insufficient improvement in target symptoms.

      • Intolerable or persistent side effects.

      • Changes in the patient's overall condition or other medical factors.

      • New clinical information or guidelines.

    • Shared Decision-Making: While Brian Thomasen NP will provide clinical recommendations based on their professional expertise, the final decision regarding medication changes will be made in collaboration with the patient and/or their adult caregivers, taking their preferences and values into account.

    • Gradual Adjustments: In most cases, medication changes will be implemented gradually to minimize potential side effects and allow for careful monitoring of the patient's response.

    • Monitoring and Follow-Up: Following any medication change, patients will be closely monitored for effectiveness and any new side effects. Regular follow-up appointments are essential during this period to assess progress and make further adjustments as needed.

  • Medication Adherence: To ensure safe and effective treatment, Brian Thomasen NP may monitor medication adherence through patient self-reports, family feedback (when appropriate), and, in some cases, pill counts or pharmacy records for controlled substances. Patients are encouraged to discuss any challenges with adherence during appointments.

Controlled Substances Policy:

  • Prescribing Practices: The decision to prescribe a controlled substance will be made after a thorough evaluation, consideration of alternative treatments, and a determination that the potential benefits outweigh the risks for the individual patient. Prescribing will be in accordance with evidence-based practices and legal guidelines.

  • Refills: Refills of controlled substances will be managed carefully and in compliance with all applicable laws.

  • Stability Requirement: As stated in our general Prescription Refill Policy, refills will generally only be provided once the patient has demonstrated stability on their current medication regimen.

  • Appointment Requirement: Routine refills of controlled substances typically require a scheduled follow-up appointment to assess the patient's ongoing need for the medication, evaluate its effectiveness, and monitor for any potential adverse effects or misuse. The frequency of these appointments will be determined based on the specific medication, the patient's condition, and regulatory requirements.

  • Early Refills: Early refills of controlled substances will generally not be provided except in very limited and documented circumstances (e.g., unavoidable travel for a brief period). Loss or theft of medication is generally not considered a valid reason for an early refill.

  • Lost or Stolen Prescriptions: Patients are responsible for safeguarding their controlled substance prescriptions and medications.

    • Reporting Loss or Theft: If a controlled substance prescription is lost or stolen, the patient should immediately notify Brian Thomasen NP and, if applicable, their local law enforcement agency.

    • Replacement Prescriptions: Replacement prescriptions for lost or stolen controlled substances will be considered on a case-by-case basis, taking into account the circumstances, the patient's history, and regulatory limitations. It is important to understand that replacement prescriptions may not be issued.

  • Adherence to Treatment: Patients prescribed controlled substances are expected to adhere to the agreed-upon treatment, including attending scheduled appointments and utilizing the medication as prescribed. Concerns about misuse or diversion will be addressed according to professional and ethical guidelines, which may include modification or discontinuation of the controlled substance prescription.

  • Medication Agreements: Patients who are prescribed controlled substances may be required to sign a separate medication agreement that outlines the responsibilities of both the patient and the provider regarding the use of these medications. This agreement may include stipulations about pill counts, urine drug screens (if deemed clinically necessary and in compliance with relevant laws), and other monitoring procedures.

  • Transfer of Prescriptions: Transfer of controlled substance prescriptions to other providers will be handled in accordance with state and federal regulations.

This policy is subject to change in accordance with evolving legal and regulatory requirements and clinical best practices. Any significant changes will be communicated to patients.

Collaboration with Other Providers:

  • To ensure comprehensive and coordinated care, Brian Thomasen NP in Psychiatry believes in collaborating with other healthcare providers involved in a patient's well-being. With the patient's (or their legal guardian's) signed HIPAA release, we may communicate and share relevant clinical information with their primary care physician, therapists, specialists, and schools (when appropriate and consented to). This collaboration helps to ensure a holistic understanding of the patient's needs and facilitates a consistent and integrated approach to their overall health and treatment. Information shared may include medication details, diagnostic information, treatment progress, and recommendations. The extent of collaboration and information shared will always be guided by the patient's best interests and the scope of the HIPAA release provided.

Substance Use Policy:

  • Brian Thomasen NP in Psychiatry primarily focuses on medication management and psychiatric evaluations for children, adolescents, and young adults. While our practice does not specialize in primary substance use disorder treatment, we recognize that substance use can significantly impact mental health and overall well-being. Therefore, our approach to patients with co-occurring substance use concerns will be as follows:

  • Integrated Assessment: During the initial evaluation and ongoing treatment, we will inquire about and assess for any current or past substance use. This information is crucial for understanding the patient's overall clinical picture and developing safe and effective treatment strategies.

  • Collaboration with Therapy: Given our requirement for concurrent therapy, we expect that the patient's primary therapist will be addressing any substance use concerns as part of their treatment plan. We will collaborate with the therapist (with the patient's signed HIPAA release) to ensure a coordinated approach to care. This may involve discussing the impact of substance use on mental health symptoms, treatment progress, and medication management.

  • Medication Management Considerations: When providing medication management to patients with substance use concerns, Brian Thomasen NP will carefully consider the potential interactions between psychiatric medications and substances of abuse. The safety and efficacy of prescribed medications will be a primary concern. We will strive to prescribe medications with the lowest potential for misuse and will monitor patients closely for any adverse effects or signs of substance-related complications.

  • Referral for Specialized Treatment: If a patient's substance use is determined to be the primary concern or requires specialized treatment beyond the scope of our practice, we will provide referrals to qualified substance use disorder treatment professionals and facilities. We believe that specialized care is essential for individuals struggling with significant substance use issues.

  • Controlled Substances and Substance Use: For patients who are prescribed controlled substances and have a history of or current substance use concerns, we will adhere to our Controlled Substances Policy, which includes careful monitoring, potential medication agreements, and consideration of the risks and benefits of prescribing these medications.

  • Patient Safety: The safety and well-being of our patients are paramount. We will take appropriate steps to address any immediate safety concerns related to substance use, which may include involving family members (with consent) or recommending crisis intervention services when necessary.

  • It is important to understand that while we will address co-occurring substance use within the context of psychiatric treatment, our primary focus remains on medication management and psychiatric evaluation. Comprehensive substance use disorder treatment requires specialized expertise and resources that may be best provided by dedicated substance use professionals.

  • Resources for Support: Patients requiring specialized substance use treatment will be provided with referrals to qualified professionals or facilities. Additional resources include the Substance Abuse and Mental Health Services Administration (SAMHSA) National Helpline at 1-800-662-HELP (4357) or https://www.samhsa.gov/find-help/national-helpline, available 24/7 for confidential support.

Emergency and Crisis Procedures:

  • Brian Thomasen NP in Psychiatry is committed to providing timely and appropriate care during scheduled appointments. However, this practice is not an emergency service. In the event of a psychiatric or medical emergency, immediate action is required.

  • If you are experiencing a life-threatening situation, have thoughts of harming yourself or others with intent, or are experiencing severe medical symptoms, please take one of the following actions immediately:

    • Call 911 for emergency medical services.

    • Go to the nearest hospital emergency room for immediate evaluation and care.

  • Please be aware that Brian Thomasen NP is not available for immediate crisis intervention outside of scheduled appointments. While we strive to provide support during our sessions, we do not offer 24/7 on-call emergency services.

    • Post-Crisis Follow-Up: After seeking emergency care, patients or their legal guardians are encouraged to notify Brian Thomasen NP as soon as possible by calling 718-218-4134 or emailing Brian@bebetterbrian.com to coordinate follow-up care and ensure continuity of treatment.

    • Patients and/or their legal guardians should provide a copy of hospital discharge documents and labs.  Families are encouraged to provide the practice email and phone number to hospital staff to ensure continuity of care and collaboration with hospital mental health providers.

Telehealth Policy:

Brian Thomasen NP in Psychiatry offers telehealth services for follow-up medication management appointments to provide convenient and accessible care. These virtual appointments are conducted securely via the Simple Practice telehealth portal. By participating in telehealth sessions, you agree to the following guidelines:

  • Technology Requirements: To participate in telehealth appointments, you will need a reliable internet connection, a device with a camera and microphone (such as a computer, tablet, or smartphone), and access to the Simple Practice telehealth portal. It is your responsibility to ensure that your technology is functioning properly prior to each appointment.  

  • Privacy and Confidentiality: Telehealth sessions are conducted on a secure platform designed to protect your privacy and confidentiality in accordance with HIPAA regulations. However, it is important for you to also be in a private and secure location during your telehealth session to prevent unauthorized individuals from overhearing or viewing the session. Please be mindful of your surroundings.

  • Appropriate Setting: For the effectiveness of the telehealth session, it is recommended that you are in a quiet and comfortable environment where you can focus on the discussion without distractions. Avoid conducting sessions in public places or while driving.

  • Emergency Protocols During Telehealth: In the event of a medical or psychiatric emergency during a telehealth session, you will be directed to call 911 or proceed to the nearest emergency room. The emergency contact information you provided during your initial intake will be utilized if necessary. Please ensure this information is accurate and up-to-date.

  • Technical Difficulties: If technical difficulties arise during a telehealth session (e.g., poor internet connection, audio or video issues), Brian Thomasen NP will make reasonable efforts to troubleshoot the problem. If the technical issues cannot be resolved promptly, the session may need to be rescheduled.

  • Scope of Telehealth Services: Telehealth appointments are primarily for follow-up medication management. Initial psychiatric evaluations are conducted in person. The appropriateness of telehealth for ongoing care will be determined collaboratively between you and Brian Thomasen NP.

  • Consent to Telehealth: By participating in telehealth appointments, you acknowledge that you understand the potential benefits and limitations of virtual care and consent to receiving services via this modality.

  • In-person follow-up appointments may be arranged on a case-by-case basis, subject to mutual agreement with Brian Thomasen NP.

Confidentiality:

  • Brian Thomasen NP in Psychiatry is committed to maintaining the confidentiality of your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and other applicable laws.

  • PHI will not be disclosed without your consent, except in specific circumstances required by law, such as mandatory reporting of suspected child abuse or neglect, imminent risk of harm to self or others, or court-ordered disclosures. These exceptions will be handled with the utmost care to protect your privacy while meeting legal obligations.

Mandated Reporting of Suspected Child Abuse or Maltreatment:

  • As mental health professionals serving children and families, we are bound by law to comply with Article 6, Title 6, Sec. 413 of the New York Social Services Law as Mandated reporters.

  • Parents or adult caregivers will be notified within 24 hours in the event of a report to the Office of Children and Family Services through the New York State Central Registry, unless such notification could compromise the safety of the child or others, as determined by Brian Thomasen NP or required by law.

Patient Rights and Responsibilities:

  • Patients have the right to receive respectful and quality care, to be informed about their treatment, to participate in treatment decisions, and to have their privacy protected.

  • Patients and their families are responsible for providing accurate information, attending scheduled appointments, adhering to the agreed-upon treatment, and paying for services as outlined in these policies.

  • Patients have the right to receive a copy of these practice policies upon request, either electronically or in print, to ensure full understanding of our services and expectations.
